This Website Privacy Policy describes information practices for vesunamd.com (the “Website”). It does not replace the Notice of Privacy Practices, which governs protected health information maintained in the practice’s health-care records.

Scope and Important Distinction

The Website contains general practice information and a “Get in touch” link. That link opens a separately hosted SimplePractice/ClientSecure contact portal. The portal is not part of the Website; its collection and processing are also governed by the portal provider’s applicable terms and privacy practices. Please do not use the Website for emergencies. If you are in immediate danger, call 911; for a mental health crisis, call or text 988.

Information Collected

The current public Website does not display a practice-hosted contact form. If you choose to use the linked SimplePractice/ClientSecure contact portal, the visible portal requests or may receive:

       Identifiers and contact information, such as your name, email address, phone number, and preferred name;

       Information you submit in connection with an inquiry, including information that may be sensitive personal information or protected health information;

       Date of birth, gender, state of residence, and whether the inquiry concerns you or another person; and

       Technical information collected by the Website host, the contact-portal provider, or other enabled service providers, such as device, browser, IP address, and usage information, as described in their own privacy documentation.

How Information Is Used

We may use information received through the Website or linked contact portal to respond to an inquiry, evaluate or coordinate a request for services, communicate about scheduling or administrative matters, protect the security of the Website and practice, comply with law, and establish, exercise, or defend legal claims. If you become a patient, information maintained in your patient record is handled under our Notice of Privacy Practices and applicable law.

The Website is publicly hosted through Google Sites. The linked “Get in touch” portal is hosted by SimplePractice/ClientSecure. We may disclose information to service providers that perform services for the practice, subject to applicable privacy and security obligations; to comply with law or lawful process; to protect rights, safety, or property; or with your consent. 

Your Choices and Requests

For protected health information in a patient record, please use the procedures in our Notice of Privacy Practices. For non-clinical Website information, you may contact us to request access, correction, or deletion where applicable. We may need to verify your identity and may retain information when permitted or required by law.

Security and Retention

No internet transmission or electronic storage method is completely secure. Please avoid sending payment card information through the linked contact portal; the portal itself instructs users not to submit payment information. We retain information only as long as reasonably necessary for the purposes described above, unless a longer period is required or permitted by law.
